Do I need a damage claim form?
Keep a damage claim form in your car in case of an accident. You never know when it might come in handy!
If the counterparty is not present, fill out your own contact information. Once you have contacted the counterparty, complete the missing information.

How do I report damage?
Reporting damage If your car is damaged, you need to report it to LeasePlan in 48 hours. You can easily report damage through My LeasePlan.
If you have insurance with LeasePlan Fill in the Mutual Statement and send the statement to the following email address: email@example.com.

With insurance from another provider Contact the insurance company's Travel Assistance department.
If this service does not exist, contact the Driver Support Line on 800 20 42 98 in Portugal (cost-free call), or +351 21 440 09 10 from abroad (national fixed-line network cost call), with the costs incurred being debited.
Damage by an unknown counterparty In case of theft or damage due to a break-in, you must report it to the authorities and obtain a police report.
Next, fill in the Mutual Statement and send it, together with the police report, to the following email address: firstname.lastname@example.org.
